Andriy Shevchenko missed a penalty, but Milan still swept through to the quarter-finals thrashing Bayern 4-1 with a Pippo Inzaghi brace.
The Rossoneri had an away goal from the 1-1 first leg result in Munich, but were far from in a comfortable position in this clash of the titans. Jaap Stam returned from injury and Johann Vogel was hoping to celebrate his 29th birthday as he stepped in for the suspended Gennaro Gattuso, while Pippo Inzaghi was chosen to partner Andriy Shevchenko upfront.
Both goalkeepers were injured during the first leg, Dida suffering his problem during the second half, but both the Brazilian and Bayern star Oliver Kahn were back in action. The San Siro was sold out with 78,569 tickets sold, including 9,000 travelling German fans.
Early on a well-time Kakha Kaladze tackle stopped Roy Makaay short and some good play between Kaka and Shevchenko led to a corner kick from Serginho's deflected cross.
Milan took the lead within the opening eight minutes, as they pounced on a defensive error. Serginho intercepted a poor Demichelis pass and crossed in for Inzaghi's cushioned header from six yards.
Bayern gave the ball away again in midfield and Vogel immediately threaded through for Inzaghi, but Kahn rushed off his line to clear the danger in a one-on-one situation.
Kahn also captured a wayward Demichelis header from Clarence Seedorf's free kick, then Vogel's volley from a corner kick was wide of the near post.
Dida's first save came on 15 minutes, comfortably blocking a deflected Bastian Schweinsteiger volley from Michael Ballack's cross, while Inzaghi couldn't make the most of another Serginho assist from the left.
On 23 minutes the referee pointed to the spot when Valerien Ismael was judged to be holding back Inzaghi. Shevchenko scored a penalty in the first leg, but this time he drilled wide of the target, so this was an opportunity wasted for the Rossoneri and Kahn tried to wind him up by shouting in his ear.
Shevchenko made up for the error just over a minute later, as the Ukrainian popped up for a glancing header on Stam's cross at the near post that slotted into the low far corner of the net.
Dida performed a fine save to push away a Deisler free kick into the centre that may have taken a touch off Ballack's head. There was a scuffle after play was halted for a foul on Inzaghi between Ballack and Kaladze.
The Rossoneri made it 4-1 on the hour mark, as Kaka showed blistering pace to spring the offside trap on Shevchenko's through ball and cannoned a powerful right-foot finish into the roof of the net with Kahn getting a fingertip to it.
Dida was brave moments later, as he punched the ball off Ballack, taking the German's chin along with it, to prevent a certain goal from six yards.
Alberto Gilardino replaced brace hero Inzaghi and Dida again only smothered a Ballack missile, but this time grabbed the loose ball before anyone could pounce.
Massimo Ambrosini made his comeback after a long-term injury for the final 15 minutes, taking over from Shevchenko for Ancelotti's Christmas Tree formation.
Seedorf burst through the centre, but tried to put it under Kahn and the goalkeeper made a good save. Pizarro's looping header settled on to the roof of the net.
Ambrosini tried to head the ball back across the face of goal for Gilardino when he could easily have gone for goal and netted a fifth.
